Location: Minnesota residence required.
 
* This is a remote position that will service our Suicide Prevention program. Qualified candidates must be licensed in the state they reside and must be within an hour’s drive of a company office.
 
Hours: 9:00 am - 5:30 pm or 10:00 am - 6:30 pm EST, Monday through Friday, local time.
 
The Behavioral Health Case Manager I/II supports our members with serious substance abuse, substance abuse-related disorders, and/or mental health-related illness and is responsible for performing case management, telephonically, within the scope of licensure for these members.
 
This position may be filled as BH Care Manager I or II - the level will be determined based on the candidate's experience and background
 
Primary responsibilities include, but are not limited to: 
  • Use appropriate screening criteria, knowledge, and clinical judgment to assess member needs. 
  • Conduct assessments to identify individual needs and develops care plan to address objectives and goals as identified during assessment. 
  • Monitor and evaluate the effectiveness of the care plan and modifies the plan as needed. 
  • Support member access to appropriate quality and cost-effective care.
  • Coordinate with internal and external resources to meet identified needs of the members and collaborates with providers.  

 

Qualifications
 
Behavioral Case Manager I Qualifications
  • Requires a current, active, and unrestricted LCSW, LICSW, LPC, LMFT, LMHC, RN, or clinical psychologist license (as allowed by applicable state law), from the state in which you reside.
  • Requires MS or MA in social work, counseling with a broad range of experience with complex psychiatric and substance abuse, or substance abuse disorder treatment; 3+ years of clinical experience, or any combination of education and experience, which would provide an equivalent background.
  • 3 years Telephonic and/or face-to-face behavioral health case management experience is required.

 
Behavioral Case Manager II Qualifications
  • Requires a current, active, and unrestricted LCSW, LICSW, LPC, LMFT, LMHC, RN, or clinical psychologist license (as allowed by applicable state law), from the state in which you reside.
  • Requires MS or MA in social work, counseling with a broad range of experience with complex psychiatric and substance abuse, or substance abuse disorder treatment; 5+ years of clinical experience, or any combination of education and experience, which would provide an equivalent background.
  • 5 years of Telephonic and/or face-to-face behavioral health case management experience is required.

 
Preferred Qualifications for Level l and ll:
Experience with community mental health resources, CSB (Community Service Board), or similar is preferred.
Experience and/or training in Suicide Prevention programs are very helpful.
Proficient in Microsoft Word, Excel, and Outlook.

  • Experience with a suicide or crisis call center - this role is 100% telephonic. They will be identifyin gpeople likely to commit suicide through algorithms, and trying to get them to join the program with Anthem. 
  • Experience with case management and connecting people to resources in the community - must reside in Minnesota - no specific city
  • Medicaid and Medicare experience beneficial
  • Personality that wants to help others 
  • Can hire into level 1 or II - Level I is 3 years of experience, Level 2 is 5 years of experience
