Essential Functions of this role:
The Clinical Care manager is responsible for assisting patients in the inpatient setting to achieve effective discharge outcomes. Through a thorough assessment of available resources and patient goals, coordinates the necessary resources to achieve the appropriate provision and achievement of patient care outcomes after discharge.

• Completes assessment of patient needs and resources within 24-48 hours of admission.
• Evaluates the allocation of services to ensure appropriate resource utilization.
• Serves as a resource to the healthcare team, communicating with physician, consultants, and all the members of the healthcare team.
• Collaborates actively and functions in a multidisciplinary department with the social worker, transitional care nurse, and the inpatient treatment team, to provide comprehensive full spectrum care coordination and discharge planning for each patient.
• Completes concurrent review of medical charts utilizing MCG, to validate the appropriate utilization of resources in patient care.
• Secures authorization for needed additional patient services, as required.
• Coordinates family and patient meetings in order to develop a comprehensive plan of care across the continuum of care delivery.

Traveler must be willing to work July 4 and Labor Day holidays, including weekends before (If Needed/Scheduled).  RTO during these holidays will not be approved. Thanks!
