The LCSW is responsible for providing behavioral health services along with a variety of care coordination services within the Internal Medicine Clinic.
These services are for patients considered high risk and who may experience barriers to healthcare and/or optimal level of function.
Services provided may include performing counseling sessions, comprehensive assessments, care planning, monitoring acute facility stays and discharges, patient education, empowering patients with self-management skills and understanding of their medical condition, identifying barriers and referring to appropriate community services.
Requirements:
- Graduation with a Master’s Degree in a mental health field.
- Possession of a valid license through the State of Colorado, Department of Regulatory Agencies (DORA) as a Licensed Clinical Social Worker.
- Current BLS Certificate credentialed by the American Heart Association, or obtained within 3 months of hire.
- Experience working with providers, clinical staff and knowledge of psycho-social community
Preferred:
- Bilingual (English/Spanish) is preferred.
SALARY RANGE
- ?$30.22 – $44.44/hour (LCSW)
- Starting wage dependent upon experience, education, and skill-set
