As an RN Case Manager, your ability to manage patient care and provide services requires substantial, specialized bedside nursing skills for symptom management, crisis intervention and family education.
In addition to excellent written/verbal communication, problem solving, and decision making abilities, the candidate should demonstrate experience with developing and maintaining rapport with patients and families, and the ability to work well with an interdisciplinary team.
Working Conditions:
Primarily works in the patient’s residence. Travel to long-term care facilities, hospitals, and area agencies and businesses will be necessary. Travel within the entire Good Samaritan Hospice service area will be necessary. Overnight travel outside of GSH service area may be necessary for continuing education. Employee is at direct risk for exposure to infections and blood borne pathogens.

- Graduate from an accredited school of nursing
- Currently licensed to practice nursing by the Virginia State Board of Nursing,
- Two years’ experience in acute care, home health, hospice, med/surg or geriatric nursing
- Current Virginia driver’s license and driving record that meets the standards set forth in current policies.
