- Within the Center for the Urban Child the “High Risk” clinic operates as a Medical Home for patients with high risk social needs that impact medical/emotional well-being.
- This social worker provides therapeutic intervention and supportive services/resources to patients and families.
- This role would also include screening of patients/families that may be identified by the larger Social Work and/or Medical team as being at high risk based on their medical and social determinants for issues such as: mental health, poverty, food insecurity, intimate partner violence, medical and physical neglect/abuse (Child Welfare agencies).
- As a member of an interdisciplinary care team, referrals are made to appropriate community/social agencies.
- This position would include, but is not be limited to, advocacy, concrete services, working with community agencies and would be instrumental in facilitating educational programming for families, and assist with some care coordination responsibilities alongside the medical staff.
- This staff member will also work in conjunction with behavioral health agencies in assessing/ providing therapeutic interventions as needed, which can include individual counseling and support groups.
- This worker would also be available for consultation and teaching to the professional community and facilitating interdisciplinary team meetings, participate/initiate research opportunities, clinic scheduling and follow up along with the members of the interdisciplinary team.
- Participation in Practice Intake coverage required.
Skills & Abilities:
Knowledge of developmental stages of children and adults. This position interacts with patients from all ages from Neonates to Adults. Knowledge of basic computer skills is an asset.
Education: Masters Degree from an accredited school of Social Work. This position interacts with patients from all ages from Neonates to Adults.
Required Licenses: LSW required
Experience: Minimum 5 years graduated experience under MSW supervision.
Supervisory Responsibility: Potential for supervising Master’s prepared Social Work interns.
